Mental Health Checklist (FREE)

Mental health affects how people think, feel, and interact with the world around them. Just like physical health, it needs care through self-awareness and simple habits, such as getting enough sleep and thinking positive thoughts.
The Mental Health Checklist is a fillable worksheet designed for clients to check in with themselves and build awareness of habits that support their mental health. This checklist can be printed for daily use or displayed as a visual guide to encourage healthy routines.
Using this checklist often can help young people recognize patterns in their mood and behavior over time. They can also write short reflections in a journal after filling it out, noting what’s going well and what they’d like to improve.
Caring adults can use the checklist as a way to start small, honest conversations. Reviewing the answers together or picking just one area to work on — like sleep or taking breaks — can make caring for mental health feel more natural.
