Short Mood and Feelings Questionnaire (SMFQ-P) - Parent Version
Welcome to the parent version of the Short Mood and Feelings Questionnaire (SMFQ-P). This screening tool has been validated for children aged 6-17. This 13-question online test can help you understand if your child might be experiencing symptoms of depression. Answering these questions will help you identify any mood-related concerns you might want to discuss with your child's doctor or mental health clinician.
How to Complete the SMFQ-P Questionnaire
Take a moment to think about how your child has been feeling and acting during the past 2 weeks. For each question, pick the answer that best describes how true each statement is for your child:
- Not True (0)
- Sometimes True (1)
- True (2)
This test asks about different aspects of mood and depression, such as feeling sad, having trouble concentrating, or losing interest in activities. Your answers will help identify if your child might benefit from professional support or evaluation.
Once you're done with the questionnaire, you can click the "View Results" button at the bottom of the page to see your answers. If you're unsure about any of the questions, you can always take the test again later.
After you finish, you can share your results with your child's doctor or mental health clinician to discuss any areas that might need attention.
