Safety Plan Worksheet
If a child or teenager is having negative thoughts and sensations indicating that they're about to go into a crisis, it helps to give them a safety plan. This includes a list of coping strategies that will help them feel better, preventing them from engaging in self-destructive behaviors.
This safety plan comes with two versions, the My Personal Safety Plan and the My Safety Plan worksheets, which allow clients to mark relevant boxes under Triggers, Signals of Distress, and write down some encouraging statements they can use whenever they need help. Furthermore, it includes boxes where they can fill out their support systems with names and contact information, plus a signature page, which solidifies an agreement to follow the plan.
If you are a parent or mental health professional, know that it's important to evaluate suicide risk and encourage them to have ownership over their safety plan.
